Functions and Responsibilities
- Understands and adheres to established policies and procedures.
- Maintains updated nursing knowledge and skills
- Assists with patient visits as needed
- Clinical record review, chart auditing
- Participate in activities associated with intake, to include maintaining positive working relationships with referral sources.
- Assist with team education development and implementation
- Support to field clinicians, PCC, DPCS
- Administrative/Clinical On call support
- Other duties as assigned by DPCS
- Attends staff meetings and/or IDG conferences and in-services as scheduled.
- Assists the patient/family in learning appropriate self-care techniques
- Must be able to use Microsoft office efficiently.
Qualifications
- Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
- At least one (1) year experience as a Licensed Vocational Nurse.
- Acceptance of philosophy and goals of this Hospice.
- Ability to exercise initiative and independent judgment with excellent communication skills.
- Completion of a Hospice training program prior to providing care.
