The Care Coordinator plays a vital role in ensuring the well-being and quality of life for individuals in our Family Care homes. Responsibilities
- Implement individualized care plans for our elderly residents based on assessments and needs.
- Coordinate with healthcare providers, family members, and employees to ensure comprehensive care delivery.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Provide education and support to caregivers regarding medical terminology, medication administration, and resident rights.
- Assist caregivers with daily living activities such as meal preparation, personal hygiene, and mobility support.
- Ensure compliance with HIPAA regulations to protect residents privacy and confidentiality.
- Maintain accurate documentation of residents interactions, care plans, and progress notes.
- Advocate for residents needs and preferences within the healthcare system.
