Pay:
- Office pay: $32.00/hr
- Visit pay (Paid per visit):
- SOC $70.00
- Recert $75.00
- ROC $75.00
- Transfer $45.00
- SUP visit $30.00
Schedule:
- Full time
- Hours / Days: M-F 8:30-5 (may need to work evenings and weekends as needed)
- Weekends: As needed
- How many hours expected to work per week: 40 - 50
Role Overview:
We are seeking an experienced Case Manager to join our team in Colorado Springs, Colorado. In this role, you will oversee and support a team of clinicians while ensuring the delivery of Private Duty Nursing (PDN) and Certified Nursing Assistant (CNA) services to clients.
Key Responsibilities:
- Hold a Colorado RN license in good standing.
- Preferably have at least one year of home health management experience.
- Provide oversight and support to clinicians and clients while monitoring pediatric and adult patient care.
- Coordinate care, assess patient needs, and ensure care plans are developed, implemented, and updated regularly.
- Monitor and review PAR documents and deadlines.
- Provide coverage for other case managers as needed.
- Administer TB skin tests.
- Perform tasks as delegated by Nursing Leadership.
- Maintain and manage patient caseloads.
- Demonstrate exceptional organizational and time management skills.
- Exhibit strong professional written and verbal communication skills.
Qualifications:
- Active RN license in Colorado.
- Ability to work a flexible schedule.
- Strong leadership skills and the ability to work autonomously.
- Highly motivated with a willingness to learn and adapt.
- A team player with the ability to navigate challenges effectively.
- Strong written and verbal communication skills.
- Willingness to travel to patient homes for visits.
