Job Description :

We are currently seeking RN IHSS Case Managers to join our team. The ideal candidates will have a strong background in home health care, a passion for serving both pediatric and adult clients, and the ability to collaborate effectively with our team of healthcare professionals.

Responsibilities :

• Provide high-quality, compassionate care to clients in their homes, ensuring their safety, comfort, and well-being.
• Collaborate with supervising RNs to develop and implement individualized care plans based on client needs and preferences.
• Conduct regular client visits within associated branch counties, demonstrating reliability and flexibility in scheduling and travel.
• Utilize Kantime software for accurate and timely documentation of client care activities and progress.
• Demonstrate knowledge and understanding of IHSS regulations and procedures to ensure compliance and quality of care.
• Maintain open communication with clients, families, and interdisciplinary team members to coordinate care and address any concerns or changes in condition.

Training Opportunities :
