ChristianaCareWilmington Hospital is looking for an energetic, motivated Nurse Manager for our Case Management Department tomanage the daily operations of the Care Management department staff and toprovide effective and efficient discharge planning services to patients inaccordance with policies, procedures, regulatory requirements consistent withCCHS.
SCOPE, PURPOSE, AND FREQUENCYOF CONTACTS:
Daily contact withPhysicians, Case Managers, Patient Financial Services, HIMS, and Managed CareOrganizations. Frequent contact with the nursing department, ancillary departments,healthcare and referring community facilities and agencies. Regularcommunication with Directors, Physician Advisors, and Social Work.
Education, Experience & LeadershipRequirements:
- Delaware RN License or compact state RN License
- Bachelors degree required.
- MSN or MBA preferred.
- Five years' experience in Case Management in an Acute Care hospital setting is preferred.
- Three years supervisory/management experience preferred.
- All candidates must be able to maintains required documented hours of relevant continuing education related to licensure requirements.
- An equivalent combination of education and experience may be substituted.
