Job Title:
We are seeking a skilled Physical Therapy Assistant to join our team.
About the Role:

- Experience in Skilled Nursing Facilities (SNFs) is preferred.
- A negative COVID-19 test result, taken within 72 hours of the start date, and issued 48 hours prior to the start date, is required.
Credentialing Requirements:
- Active state license verification
- Up-to-date resume
- Three professional references
- County criminal background check for all counties lived and worked in (7 years)
- Federal criminal background check (7 years or more)
- Multistate criminal background search (7 years or more)
- Nationwide sex offender search (7 years or more)
- HHS/OIG search
- GSA search
- Social Security Number trace
- Proof of positive PPD and chest X-ray (or alternative testing methods)
- SAM search
- CORI Massachusetts background check
- 8 hours of dementia training
Work Schedule:
Normal shifts: Monday through Friday, with day shifts available.
Benefits:
- Minimum guaranteed hours: 32 per week
- Contract length: 13 weeks
