- Providing visits as assigned by the Clinical Services Manager, including assessment, determination of eligibility, admissions processing, planning, implementation, and evaluation phases of the nursing process.
- Assigning tasks and coordinating interventions with other members of the interdisciplinary team.
- Maintaining accurate patient records, charts, progress notes, reports, and preparing care plans timely.
