As a Home Health Physical Therapist, Your Responsibilities Will Include :
Delivering physical therapy services as outlined in the physician's plan of care and state Physical Therapy Practice Act.
Consulting and educating clients, their families, and team members on treatment procedures and activities.
Assisting in the evaluation of clients' levels of function and contributing to the care plan development.
Preparing detailed clinical and progress notes.
Translating exercises into functional activities or daily living skills.
Adapting equipment and surroundings to meet client needs.
Regularly updating the attending provider on client progress and conditions.
Communicating treatment implications and supporting family engagement in the process.
Providing encouragement and information to families to assist their progress.
Submitting clinical summaries and progress reports based on client goal achievement.
Supervising physical therapy assistants in accordance with policy and regulations.
Maintaining accurate clinical records to track client goals and progress.
Qualifications :
Current licensure as a Physical Therapist.
Graduation from an accredited School of Physical Therapy recognized by relevant associations.
Preferred experience of at least two (2) years in an acute hospital or similar setting.
