Qualifications :
Master's Degree in Social Work required for New Mexico.
Current State Licensure in Social Work (LMSW or LCSW in New Mexico).
Minimum of one year’s experience in a healthcare or hospice setting.
Valid driver's license and proof of insurance are required.

Your Role :
Conduct timely psychosocial assessments for patients and families involved in our hospice program, ensuring their needs are addressed in the care plan.
Deliver exceptional service and support to patients, families, and team members at all times.
Provide counseling to patients, families, and staff concerning stress and coping challenges.
Document patient services accurately and in accordance with the plan of care.
Assist with applications for Supplemental Security Income, Medicaid, and more to help meet patient and family needs.
Act as a liaison between families and community agencies to facilitate support and resources.
