Position Summary : This position will be responsible for will conducting in-home visits at clients' homes to monitor and report on the quality of services being provided by Home Care Aides, and to determine whether existing care plans are adequate to meet client needs.
Location : Ambercare 2708 N Prince Suite A Clovis, NM 88101-9391
Schedule : Monday Through Friday - 8am - 5pm. 1 hour lunch. Some on-calls weekends

Essential Duties :
Monitors clients condition and performance of duties by Home Care AidesMaintains all company and Department on Aging rules, regulations, and standardsConducts home visits to new clients to welcome and review the Welcome PacketObserves and evaluates Home Care Aides in the client's home as directed by Supervisor, to assure client Care Plans are being followedPrepares and completes accurate evaluation reports and case notes as neededMaintains schedules to assure timely completion of all assigned home visitsAttends weekly branch meetingsMaintains a high degree of confidentiality at all times due to access to sensitive informationMaintains regular, predictable, consistent attendance and is flexible to meet the needs of the departmentFollows all Medicare, Medicaid, and HIPAA regulations and requirementsAbides by all regulations, policies, procedures and standardsPerforms other duties as assignedPerformance Responsibilities :
Maintain positive internal and external customer service relationshipsMaintains open lines of communicationPlans and organizes work effectively and ensures its completionMeets all productivity requirementsDemonstrates team behavior and promotes a team-oriented environmentActively participates in Continuous Quality ImprovementRepresents the organization professionally at all timesPosition Requirements & Competencies :
Minimum of 3 years experience working in the field of senior services, or related social serviceCNA certificate and license preferred; or similar trainingMust demonstrate strong interpersonal and communication skillsRepresents the organization professionally at all timesMust be client focused and customer service drivenComputer experience Microsoft Word and ExcelMust have reliable transportation, valid drivers license and insurance for travel within the branch service areasMust be bilingualTo apply via text, text 7331 to 575- 205-1296
