If so we’re looking for you Ambercare Home Health, an Addus HomeCare Company is an industry leader and currently searching for a PRN Transitional Care Coordinator – Licensed Practical Nurse based in Albuquerque, NM. 3-4 month Commitment of at least 30 hours a week Ongoing PRN work after the initial period. Mileage reimbursement What You’ll Do : Clinically manage clients enrolled in value-based programs Work as a facilitator between the various internal departments to ensure product policy and processes are documented and executed accurately Serve as the front-line clinical support, including performance of home visits, to coordinate care between the client and various clinical team members. Responsible for pre-empting potential problems and working with the team to develop effective solutions Responsible for collecting, processing and reporting data to internal and external customers Identify requirements for product implementation Assist leadership to implement, oversee, and support Value Based products Requirements : Current LPN license with at least three (3) years of experience in a clinical setting Previous Experience in home care and home health. Knowledge of managed home healthcare is preferred Candidate must possess excellent verbal and written skills Solid computer proficiency in Microsoft Office including Excel and web based documentation applications (e.g Home Care Home Base, Epic, E-clinical etc).
