Job Description

Job Title : Physical Therapist

Job Specialty : Home Health

Job Duration : 13 Weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty

License : Must have a license in hand

Certifications : CPR, BLS

Must-Have :

Reliable transportation, current driver's license, and agency-required liability insurance
Proficiency in HomeCare HomeBase EMR, used within the last 6 months; if not, a retraining course will be required
Proficiency in Oasis for RNs, PTs, and OTs

Job Description :

Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.

Responsible for home visits with periodic office meetings.

Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Average of 5-8 patients per day with points assigned per visit and activity, totaling 30 points per week.

Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits.

Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.

Local candidates accepted at the same rate.

Provide care in the designated territory or county.

Cover DeQueen and the surrounding area.

Meet weekly productivity expectations of 30 points, equating to 40 hours per week.

Understand that there are no guaranteed hours with this contract.

Ensure documentation is completed within 24 hours of the appointment.

Be open to traveling to nearby facilities if patient census is low.

Participate in home health checklist and be open to on-call rotations if required.
