Job Description

Job Title : Physical Therapist Home Health

Job Specialty : Home Health Physical Therapy

Job Duration : 13 Weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year in Adult Home Health

License : Must have a valid license in hand

Certifications : CPR, BLS

Must-Have :

Recent Home Health experience
Proficient in HomeCare HomeBase EMR

Job Description :

Conduct home visits to patients averaging 5-6 per day

Maintain reliable transportation with a valid driver's license and required auto insurance

Attend periodic office meetings

Achieve a minimum of 30 productivity points per week, which include patient visits and other activities

Cover a travel radius of up to 75 miles from the home clinic, with mileage reimbursed

Address different types of cases such as Start of Care and Follow-Ups

Complete all documentation within 24 hours of a patient appointment

Be open to floating to a nearby clinic if patient availability is too low to meet financial needs

Work Monday to Friday from 8 am to 5 pm, with no weekend or on-call rotations required

Territory primarily includes New Castle County

Use the HCHB EMR system for patient records and management

Ensure you are prepared for travel, patient diversity, and necessary productivity requirements in this role.
