Job Description

Job Title : Licensed Vocational Nurse / Licensed Practical Nurse

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

  • Proficiency with HomeCare HomeBase (HCHB) within the last 6 months or willingness to complete a retraining course
  • Ability to travel up to 75 miles from the home clinic
  • Reliable transportation, driver's license, and agency-required liability insurance

Job Description :

  • Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures through competent application of the nursing process.
  • Responsible to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
  • Conduct home visits and attend periodic office meetings.
  • Manage an average of 5-8 patients per day, equating to roughly 6-8 points, with a weekly productivity expectation of 30 points.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • No guaranteed hours, with the opportunity to float to a nearby clinic if necessary.
  • Expect to take on-call duty and work varying weekend rotations as needed.
  • Complete all documentation within 24 hours of patient appointments to ensure proper billing and follow-up patient care.
  • Adherence to dress code requirements, wearing scrubs.
  • Familiarity with documentation systems is essential, specifically HomeCare HomeBase.
