Job Description

Job Title : LPN Home Health

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase EMR, used within the last 6 months, or completion of a retraining course
Ability to travel up to 75 miles from the home clinic
Reliable transportation, driver's license, and auto insurance
Comfortable with drives that may be longer than 20 minutes between patients
Open to traveling 150-250 miles per week
Willingness to accept every patient on the schedule regardless of distance
Execute all documentation within 24 hours of patient appointments

Job Description :

Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through competent application of the nursing process

Responsible for supervising and evaluating Home Health Aides and other Licensed Practical / Vocational Nurses

Conduct home visits and periodic office meetings

Manage an average of 5-8 patients per day with points assigned per week

Handle cases such as Start of Care, Wound Care, and Follow-up Visits

Participate in monthly weekend rotations, and be available for on-call duties if needed

Complete documentation promptly to ensure timely billing and follow-up care

Be prepared to float to nearby clinics if the patient census is low
