Job Title : Therapy PTA Home Health
Job Specialty : PTA Home Health

Shift : Day, 5x8-Hour (08 : 00 - 17 : 00)

Experience : Minimum 1 full year Adult Home Health Experience
License : Must have current drivers license and agency-required liability insurance
Certifications : CPR BLS
Must-Have : Proficiency in HomeCare HomeBase (HCHB) and experience with Oasis for RNs, PTs, and OTs.
Job Description :
- Conduct home visits for 5-8 patients per day
- Attend periodic office meetings
- Reliable transportation and liability insurance required
- Responsible for Start of Care and Follow-Up cases
- Travel within a 75-mile radius from home clinic
- Proficiency in HomeCare HomeBase (HCHB)
- Documentation to be completed within 24 hours of each appointment
- Willingness to float to nearby clinics if patient census is low
- No guaranteed hours, must meet weekly productivity points
- Must accept every patient on schedule regardless of distance between appointments
