Job Description

Job Title : Physical Therapist Assistant (PTA)

Job Specialty : Home Health

Job Duration : 13 Weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : Not guaranteed

Experience : Minimum 1 full year of adult home health experience

License : Must have license in hand prior to starting

Certifications : CPR, BLS

Must-Have :

  • Reliable transportation
  • Current driver's license
  • Agency-required liability insurance

Job Description :

  • Conduct home visits and participate in periodic office meetings
  • Assigned 5-10 patients per day, equating to roughly 6-8 points
  • Average of 5-6 patients per day
  • Responsible for Start of Care and Follow-Up cases
  • Travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate
  • Use of HomeCare HomeBase EMR; proficiency required
  • Documentation must be completed within 24 hours of the patient appointment
  • Must be comfortable with travel and scheduling flexibility, including potential for long drives between patients
  • Open to on-call rotations expected
  • No guaranteed hours; must achieve weekly productivity points of 30, equivalent to 40 hours per week
  • If patient census is low, floating to nearby clinics for additional hours may be needed
  • Orientation provided at the beginning of the assignment
