Job Description

Job Title : Physical Therapist - Home Health

Job Specialty : PT Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : Not guaranteed

Experience : Minimum 1 full year Adult Home Health Experience or Hospice experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

  • Proficient in HomeCare HomeBase EMR and used it within the last 6 months, or must complete a retraining course during compliance.
  • Proficient in OASIS (if applicable for related professions).
  • Reliable transportation with a valid driver's license and auto insurance.
  • Ability to travel up to 75 miles from the home clinic.
  • Previous home health experience.
  • Comfortable with the possibility of on-call rotations.

Job Description :

  • Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
  • Responsible for the supervision and evaluation of Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
  • Conduct home visits and attend periodic office meetings.
  • Manage an average of 5-8 patients per day, collecting roughly 6-8 points through various activities.
  • Handle various cases, including Start of Care, Wound Care, and Follow-up Visits.
  • Complete documentation within 24 hours of the patient appointment.
  • Maintain productivity expectations of 30 points per week.
  • Visit patients in their homes and assisted living facilities (ALFs).
  • Be open to floating to nearby clinics if necessary to meet work requirements.
