Job Summary
Amedisys Home Health Care is seeking an experienced RN Home Visitor to join our team. As a key member of our staff, you will provide high-quality home health care services to patients, focusing on improving their quality of life and promoting optimal health outcomes.
Main Responsibilities:
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement personalized care plans based on patients' unique requirements
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Monitor patients' progress and adjust care plans as needed
- Maintain accurate and detailed records of patient interactions and care provided
Required Skills and Qualifications:
- Minimum 1 full year of adult home health experience or 1 full year of hospice experience
- Recent specialty experience required
- Current BLS certification
- Ability to travel up to 75 miles from home clinic
- Proficiency in HomeCare HomeBase (HCHB) and Oasis software
