The Day Habilitation Program Coordinator (PC) will evaluate, develop, implement and supervise the implementation of the Dayhab service plan (DHSP), as well as review the member's plan. The PC will ensure plan updates are made to the DHSPs by PCs. The PC will participate in interdisciplinary team meetings and maintain member records in accordance with regulatory standards. The PC provides for member service needs, member goals/objectives and program objectives and acts as a resource for members, families, team members, and other stakeholders. Duties will include participation in member and group facilitation and program activities, within all environments; training and implementation of member behavior support plans, direct care responsibilities and other duties as assigned. Under the direction of the Director of Adult Day Services, the PC must participate in finding and maintaining meaningful community-based activities that the individuals can enjoy. They are responsible for the overall management functions which include working with PCs for planning, implementation and assessment of services to ensure that individuals needs are being met and that services are of high quality within the program. Creativity in securing meaningful volunteer and community-based activities is instrumental to the success of the services delivered.
Primary Duties Include:
- Assist with management of the Day Habilitation Program in such a manner as to ensure that support services and community inclusion opportunities are maximized for individuals receiving services. This includes ensuring group activities are routinely planned and carried out and that staff are trained in delivering appropriate developmental activities to individuals receiving services.
- Provide positive, innovative and effective leadership and motivation within your department to ensure that our organization is a responsive and professional organization, that it carries out our mission well and that our stakeholders perceive it as such. This includes periodic physical spot checks of all outside work sites and/or volunteer sites.
- Verbal spot checks with employers and/or volunteer organizers should be done periodically as well. Documentation of all spot checks should be maintained. This is to ensure the proper delivery of servicesfor both the work site and the volunteer site. The minimum is 12 checks a month. This includes regular supervisions, staff development and mentoring, as well as managing employee relation issues that arise. Ensure that disciplinary issues are addressed within the parameters of the Progressive Discipline Policy and in coordination with Human Resources/Labor Relations.
- Assists with in-take and orientation process for new members; ensures Interim Day Hab plan is completed within 5 business days of member admittance to program; ensure all DHSP documentation including service assessments and goals/objectives are completed within the 45 business days of admittance to program.
- Evaluate member supports to determine service needs with emphasis on development of skill areas and expressed preferences.
- Participates in the development of the Day Habilitation Service Plan, including goals/objectives, provider support strategies and data collection systems. Supports DHSM with this process.
- Ensure implementation of members DHSP goals/objectives and corresponding data collection. Train DHSM, Developmental Specialists and paraprofessionals on member, group and program activities in accordance with respective schedules, goals and objectives, and reinforce a team approach with open communication, support and instruction.
- Provide support for all therapy staff including OT, PT, SLP, and BCBA as needed.
- With relevant program staff, evaluate clinical and programmatic needs. Ensure implementation of appropriate curriculum so as to address individual needs within the context of available resources.
- Work with Program Director to ensure that the Day Habilitation Department operates within established budget guidelines.
Other duties as assigned.
Pay Range: $48,000-$55,000 based on education and experience.

Bachelors Degree preferred. Must have 3 years leadership and management experience in a human service
Demonstrated experience in change management and innovation, strategic planning, financial management, organizational growth and expansion.
Effective communicator, with experience developing decision-making processes as a facilitator.
