The Outreach Care Specialist is responsible for ensuringthat appropriate member treatment plans are followed and identifies ways toimprove the member’s health and quality goals.
Minimum Requirements:
- High school diploma preferred
- One year of related experience preferred
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
