- Provide primary care services to adults, including family medicine, diabetes prevention/management, and chronic/acute care.
- Collaborate with other healthcare providers to ensure seamless care coordination and referrals to off-site specialists when necessary.
- Manage a caseload of patients, prioritizing their needs and developing personalized care plans.
