Geriatric Primary Care Nurse Practitioner
We are seeking a Geriatric Primary Care Nurse Practitioner to work 4 days a week at our clinic in Union City, CA. The ideal candidate will have experience working with patients with chronic illnesses and be available for telehealth visits.
Job Details:
- Type: Part-time/Full-time position (varies)
- Shift: Monday, Tuesday, Thursday, Friday - 7am-4pm
- Duty Hours: Minimum 32 hours per week
Responsibilities:
- Provide primary care services to geriatric patients
- Conduct thorough assessments, develop treatment plans, and coordinate care
- Manage chronic illnesses and perform routine check-ups
- Prioritize patient needs, communicate effectively with colleagues and medical staff
Requirements:
- Nurse Practitioner (NP) licensure in the state of California
- Experience working with geriatric patients and chronic illnesses
- Strong communication and interpersonal skills
Compensation:
$120,000 - $150,000 annually based on experience
