Key Responsibilities:
- Hospice/Palliative Care Services:
- Work independently as an Advanced Practice Provider (APP) to deliver compassionate care to patients and their families.
- Collaborate with the Palliative Care Medical Team to assess patient needs, develop care plans, and provide ongoing support throughout the patient's journey.
- Clinical Expertise:
- Prescribe medications and order lab tests as needed.
- Conduct thorough assessments to identify disease-specific changes, pain management needs, and other distressing symptoms.
- Develop and implement individualized treatment plans to improve patient quality of life.
- Communication and Collaboration:
- Maintain open communication with the Palliative Medical Director and multidisciplinary care team.
- Participate in regular meetings to discuss patient progress, challenges, and best practices.

Requirements:
- Nursing Experience: Minimum of 2 years of experience as a Nurse Practitioner.
- Certification: ACHPN certification preferred.
