About the Job
This role involves working directly with residents, providing training and assistance with activities of daily living, such as personal care, meal preparation, and household chores. You will also be responsible for reporting any changes in status to Team Leaders, Program Coordinators, or Program Directors, and documenting care accurately according to company policies and procedures.
Key Responsibilities
- Provides training and assistance to assigned individuals with interpersonal skills and activities of daily living in accordance with their individual support plan.
- Report any changes in status to Team Leader, Program Coordinator, or Program Director.
- Document care accurately per company policies and procedures, turning documentation in on a timely basis.
- Participate in multi-disciplinary team conferences and in-services as requested.
- Provide therapeutic support in daily activities.
- Perform other duties as assigned or necessary.
- Accompany residents to scheduled activities both inside and out of the facility/home.
- Contribute to revisions of individual service plans as requested by client, team leader, or program coordinator.
- Assist with admissions, discharges, and transferring of residents.
- Support independence by assisting with routine household chores and other ADLS.
Requirements
- High School diploma or equivalent required.
- Complies with all relevant professional standards of practice.
- Completion of Amergis Competency program when applicable.
- CPR certification if applicable.
- TB Questionnaire, PPD, or chest x-ray if applicable.
- Current Health Certificate (per contract or state regulation).
- Meets federal, state, and local requirements.
- Successful completion of new hire training.
- Understands patient confidentiality and HIPAA requirements.
- Effective communication skills and proficiency in English required.
- Computer proficiency required.
- At least 18 years old.
- Prioritizes tasks effectively.
