ACC supports and uplifts the community through accessible and compassionate behavioral health services.Position Summary : ACC is seeking a dedicated Behavioral Health (BH) Case Manager to join our Detroit team. This role is responsible for coordinating comprehensive care for clients with behavioral health needs by assessing, planning, implementing, monitoring, and evaluating services. The BH Case Manager will work closely with clients, families, and healthcare providers to ensure effective, recovery-oriented, and client-centered support.Qualifications : Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field.At least one year of experience in behavioral health or case management preferred.Ability to work with diverse populations and handle complex cases with empathy and professionalism.Demonstrate strong written, verbal, and interpersonal communication skills during client interactions.Employ critical thinking and situational awareness to provide effective case management.Capacity to travel to client locations using a personal, reliable vehicle; maintain a valid driver's license, registration, and insurance.This position does not require bilingual proficiency, but knowledge of the Arabic language would be beneficial.DutiesJob Duties : Comprehensive Client Assessment and Care PlanningDevelop and document individualized care plans that address identified needs and set achievable goals in collaboration with clients, their families, and other healthcare providers.Care Coordination and Service IntegrationCoordinate the delivery of behavioral health services across various providers, including medical, psychiatric, and social services.Facilitate communication and collaboration among all parties involved in the client's care to ensure integrated and continuous support.Monitor and manage transitions of care to prevent gaps in service delivery and to ensure smooth transitions between different levels of care.Client Advocacy and Resource ConnectionAdvocate for clients to ensure they receive appropriate services and resources, including financial, housing, vocational, and educational support.Connect clients with community resources and support networks that enhance their ability to manage their behavioral health conditions and improve their quality of life.Assist clients in navigating healthcare systems and overcoming barriers to accessing necessary care and services.Monitoring and Evaluating Client ProgressRegularly review and update clients' care plans based on their progress and changing needs.Conduct ongoing assessments to track clients' progress toward their goals, adjusting interventions as necessary to achieve optimal outcomes.Maintain accurate and timely documentation of all client interactions, assessments, care plans, and progress notes.Crisis Intervention and SupportProvide immediate support and intervention in crisis situations, utilizing appropriate de-escalation techniques and crisis management strategies.Develop and implement crisis prevention plans to reduce the risk of future crises and to promote client stability and safety.Offer emotional support and counseling to clients and their families during times of crisis, helping them to cope with stress and navigate challenging situations.Complete all additional tasks and duties as assigned by direct manager.Perks & Benefits : Guaranteed full-time pay - competitive salariesFull medical, dental, vision health benefits with low costs and deductiblesShort-term and long-term disability insuranceGenerous paid time off (20 days a year)14 paid holidays (Your Birthday is a paid holiday!)403(b) Retirement PlanBonus opportunities- employee referralIf you are passionate about behavioral health and committed to helping individuals achieve their full potential, we encourage you to apply!ACC is an equal opportunity employer.
