Job Overview:
We are seeking an experienced and dedicated Licensed Practical Nurse (LPN) to join our team. The IDD LPN will provide skilled nursing care to patients with intellectual and developmental disabilities in their homes. You will collaborate with a compassionate team to ensure that each patient receives the personalized care they deserve, improving their overall well-being and quality of life.
Responsibilities:
- Provide nursing care to patients with intellectual and developmental disabilities, including medication administration, wound care, and monitoring of vital signs.
- Collaborate with a multidisciplinary team to create and implement individualized care plans.
- Educate patients and families on managing their care needs, treatment plans, and medication.
- Assist with daily activities and ensure a safe, comfortable environment for patients.
- Maintain accurate patient records in accordance with company policies and regulatory standards.
- Monitor patients’ conditions and report any significant changes to physicians and management.
- Monitor ventilator settings and parameters, assessing respiratory and hemodynamic status, evaluating patient comfort and safety, and monitoring for complications.
- Safely perform nasal, oral, pharyngeal, and tracheostomy suctioning.
- Ensure compliance with all state and federal regulations related to patient care.
Qualifications:
- Valid LPN license in the state of employment.
- Minimum of 1 year of nursing experience, preferably in home health care or with IDD populations.
- Minimum of 2 years working with clients on ventilators and/or tracheostomy
- Compassionate, patient, and empathetic demeanor with the ability to build strong rapport with patients and families.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexible and adaptable to changing schedules and patient needs.
