What are the responsibilities and job description for the Care Manager position at American Association of Caregiving Youth?
Purpose of the Position:
To recognize, support and promote the awareness of dual role young people who are both students and family caregivers; to maximize the outcomes of student-caregivers and their families by work directly with them and in schools and the community; to assess needs and make special project recommendations and appropriate community referrals, to assist the Director of the Caregiving Youth Project and other staff as needed.
Goals of the Care Manager:
- Establish and maintain relationships with caregiving youth and their families, schools and the community.
- Conduct in-home family assessments with utilization of resources and follow up visits as needed.
- Work within a team environment to foster program development and expansion, along with student and family satisfaction.

North Assistant Director
Essential Responsibilities:
- Collaborates with CYP staff for the identification and ongoing needs of youth caregivers and their families
- Conducts in-home family assessments to identify and address psycho-social, educational, medical, and economic challenges of individuals, families, and their formal and informal social/support systems
- Uses resources and knowledge of chronic conditions and preventive measures from and translates that information into an understandable format for youth and their families from which to learn and benefit
- Assists Activity Manager with implementing and planning out-of-school recreational respite activities
- Initiates, maintains and documents contact with CYP students and their families
- Communicates regularly with staff, students, families and referral sources to ensure linkages with and utilization of resources
- Refers to Family Specialist and/or CYP Director any concerns regarding CYP student and family needs (academic, health, mental health, social emotional, physical, etc.)
- Maintains timely documentation according to best practice standards and organization policy
- Contributes to ongoing development of the CYP program including program expansion and recommendations
- Performs a range of administrative, reporting, educational, and advocacy functions on behalf of youth, their families and CYP
- Participates in camps and events
- Engages in fund raising and development activities with volunteers, staff and the community
- Drives CYP youth to and from activities as needed
- Participates in community initiatives that foster project development including the opportunity to make presentations and educational programs
- Seeks opportunities for CYP to increase public awareness of the issues that caregiving youth face
- Performs other responsibilities as requested.
Qualifications:
Bachelor’s degree in psychology, social work or a social service professional with demonstrated expertise in Care Management and family support.
- Speaks and writes additional languages; Spanish REQUIRED and Creole helpful; seeks resources to bridge communication gaps and language skills challenges as needed;
- Uses age appropriate active listening skills to determine the direct or indirect question being asked and therefore its answer(s);
- Has experience working with middle school and high school youth from diverse cultural and socioeconomic groups;
- Possesses excellent verbal and written communication skills;
- Presents personable professional appearance.
Work Location:
Time will be spent in the AACY office, in client homes and other agency and activity related locations.
Job Type: Part-time
Pay: $31,590.00 per year
Expected hours: 20 – 25 per week
