What are the responsibilities and job description for the Hospice Medical Social Worker,MSW position at American Care Hospice?
The Social Worker will assist patients and families with personal and environmental difficulties that interfere with obtaining maximum benefits from care. Additionally, the MSW will assist the physician and other members of the health care team in understanding significant social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.

· Master degree in social work (MSW) required.
MSW Main Duties:
Ø Performs initial comprehensive and ongoing psychosocial and bereavement assessments in a timely manner.
Ø Provides individual, family, and/or group counseling to the terminally ill, their families and bereaved within expected scope of practice and as needed.
Ø Develops, implements, documents and revises the plan of care for psychosocial concerns as needed in a timely manner, in conjunction with the IDG.
Ø Maintains Department of Health, HIPAA and other regulatory compliance with documentation, plan of care and delivery of services.
Ø Prepares for and participates in IDG, promoting inter-disciplinary collaboration and addresses psychosocial interventions.
Ø Develops and maintains contact with appropriate community agencies and has knowledge of community services in order to promote inter-agency cooperation and to facilitate related referrals.
Ø Provides education to staff, volunteers, and community on hospice social work role and death and dying issues. Participates in the orientation of new members of the IDG and conducts hospice training on psychosocial issues as needed.
Ø Completes all documentation in accordance with the standards.
Experience Medical Social Work in a hospital, long term care acute care setting preferred.
