SALARY: $31-$36 per hour
Job Description:
The Intake Coordinator is responsible for the admission process for new patients by ensuring the availability of reimbursement for services, identifying the clinical team to deliver care, and scheduling all patient visits within the prescribed discipline and visit frequency requirements.
Responsibilities:
- Receives referrals from physicians and facilities and initiates the intake process.
- Coordinate care by assigning a case manager and clinician to ensure 48-hour admission.
- Enters new patient records in the system ensuring the accuracy of all patient demographic information.
- Performs eligibility/insurance verification.
- Perform follow-up calls to patients and families to offer clear and supportive communication.
- Perform other various coordination duties to support day to day workflow.
Qualifications:
- 1 year: Intake coordinator experience in Home Health or Hospice
- 2 years: Experience in healthcare
- Must be able to multitask
- Computer literacy: able to work with data on multiple screens, using different software programs
- Goal oriented, proactive and great attention to detail
