Description:
We are a small and well-organized home health agency in Miami-Dade looking for a bilingual (English/Spanish) Registered Nurse Case Manager for an office-based role.
This position is perfect for a retired or semi-retired RN who still wants to stay active in the profession but in a calm, supportive, and low-stress environment. No night shifts, no weekends, and no on-call duties. You'll mostly work in the office reviewing documentation, coordinating care, and supporting field staff.
Key Responsibilities:
- Coordinate and oversee patient care plans
- Communicate with physicians, caregivers, and field staff
- Review clinical documentation for compliance and quality
- Support quality improvement and patient follow-ups
Schedule:
- Monday to Friday (regular business hours)
- No weekends, holidays, or on-call
- Primarily office-based with occasional field coverage if needed
Requirements:
- Active RN license in Florida (required)
- 1 year of home health or case management experience preferred
- Fluent in English and Spanish (required)
- Familiar with Medicare home health regulations
- Comfortable learning EMR systems (Axxess)
