This role involves independent management of a caseload, utilizing advanced counseling, assessment, and decision-making skills to address the unique needs of each individual or family. The Chaplain ensures that the emotional and spiritual needs of patients are met while collaborating with interdisciplinary teams to promote holistic care.

20 - $30 an hour
30- $40 per visit
American Hospice and Home Health Services is seeking a Part-Time Spiritual Chaplain / Counselor to serve in Napa County. Negotiable salary and hourly pay rates depending on experience.
Responsibilities and Duties :
Patient and Family Care :
- Provides spiritual and emotional support tailored to the beliefs and preferences of patients and families.
- Conducts initial and ongoing spiritual assessments to identify needs and develop individualized care plans.
- Offers grief counseling and bereavement support to families and caregivers during and after a patient's journey.
- Acts as a liaison between the patient's faith community and the care team, when requested.
- Coordinates with the interdisciplinary team to address spiritual and emotional goals in the overall care plan, in a meeting every other week.
