Admissions Social Worker - Nursing Home
Located in Freeport, NY
Salary: $70K - $80K; Based on Prior Experience
Qualifications:
- Must have experience as a Long-Term Care Social Worker
- Nursing home documentation including MDS, care plans, progress notes
- Must have BSW or MSW
- Advocating for and protecting resident’s rights
- Ensuring the social and emotional needs of each resident are met
- Must have excellent outreach, assessment, written and verbal communication skills.
- Must be able to work effectively as part of a team.
- Knowledge of SigmaCare, a plus
Responsibilities:
- Meet and greet every new family and resident
- Complete all necessary admission paperwork and assessment in a timely fashion
- Work with residents and families on advanced directives
- Effectively communicate with interdisciplinary team
- Create and update Care Plans
- Review and/or establish advanced care directives.
- Psychosocial assessment
