Salary: Up to $120K ; Based on prior experience

Qualifications:

• Must have New York RN license
• Must have prior experience as a Unit Manager
• Must have long-term care experience
• Must have strong management skills
• Ability to work with a variety of health care professionals and patients

Job Responsibilities:

• Conduct resident assessments to develop individualized care plans.
• Monitor resident health status and make necessary adjustments to care plans as needed.
• Ensure compliance with all federal and state regulations regarding resident care.
• Regularly update families on resident progress and address any concerns.
• Facilitate family meetings to discuss care plans and decision-making.
• Prepare and maintain accurate resident documentation.
