RN Home Visit Specialist Job Description
AME Home Care LLC is seeking a highly skilled Registered Nurse (RN) to provide home visit services to seniors and individuals with disabilities. As an RN Home Visit Specialist, you will conduct comprehensive assessments, develop personalized plans of care, and provide education and training to clients and their families.
Key Responsibilities:
- Conduct thorough assessments of clients' physical and emotional needs.
- Develop personalized plans of care that meet clients' unique needs and goals.
- Provide education and training to clients and their families on health-related topics.
Requirements:
- Current, unencumbered state license to practice as a Registered Nurse (RN).
- Minimum one year of nursing home care/home health experience required.
- CPR/First Aid certification and reliable transportation.
- Copy of updated Physical based on state requirements and clean criminal history.
