- Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
- Develop and implement individualized care plans, focusing on pain management and improving patients' quality of life.
- Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
- Educate patients and their families on treatment plans, health maintenance, and preventive care.
- Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
Qualifications:
- Licensed Nurse Practitioner with a valid Ohio certification.
- Experience in skilled nursing facilities or a similar environment is preferred.
- Strong knowledge of pain management techniques and treatment plans is preferred.
- Excellent communication and interpersonal skills, with a genuine passion for helping others.
- Ability to work collaboratively with a multidisciplinary team.
