Role Summary:
The Registered Nurse will be responsible for providing high-quality patient care to individuals with End Stage Renal Disease (ESRD). This includes assessing patient needs, developing individualized care plans, and collaborating with interdisciplinary teams to optimize patient outcomes.
Key Responsibilities:
- Assess patient needs and develop individualized care plans.
- Provide high-quality patient care, promoting a positive and supportive environment.
- Collaborate with interdisciplinary teams to ensure seamless care transitions.
- Document patient progress and adjust care plans as needed.
Requirements:
- Associate Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (BSN); three-year diploma from an accredited diploma program may be substituted for nursing degree.
- Registered Nurse License in the state of practice.
- Current BLS/CPR certification required.
- Meets all applicable state/licensing requirements.
- Previous dialysis, ICU, CCU, ER, or Med Surg nursing experience is preferred but not required.
