Position: PRN Field Registered Nurse (RN)
We are currently seeking a PRN Field RN to provide home health and hospice care in PLAINVIEW (TEXAS), and surrounding areas.
- Coverage Area: 60-mile radius around Lubbock (South, East & West locations)
- Compensation: $35 - $80 per visit (not per hour; rate depends on the type of visit)
- Experience: Home Health & Hospice experience is preferred
Responsibilities:
- Provide home health and hospice care visits as required.
- Conduct Start of Care, Resumption of Care, Re-certification, Discharge, and other patient visits.
- Ensure patient charts are completed and submitted in the EMR system within the required timeframe.
- Perform assessments and reassessments when there is a significant change in a patient's condition, at a physician’s request, or after hospital discharge.
- Consult with supervisors, staff, and families regarding patient care plans and services.
- Maintain effective communication with clinical teams and leadership.
- Direct clinical staff as needed and coordinate patient care with other program areas.
- Support quality improvement initiatives and timely implementation of corrective actions.
- Maintain proper documentation and ensure compliance with regulations.
- Perform other duties as assigned by the company.
Job Requirements:
- Active Registered Nurse (RN) license in Texas.
- Prefer, at least one year of experience in home health and/or hospice care.
- A Bachelor’s degree in nursing is preferred but not required.
- Strong verbal and written communication skills.
- Physical capability to perform assessments and provide patient care.
Work Conditions:
- The role involves traveling to patient homes and other facilities
- Some physical activity such as bending, stooping, and lifting may be required.
- Must be able to access patients' homes, which may not always be wheelchair accessible.
- High volume of telephone and documentation work.
