We are seeking an energetic Community Community Support Colleague that can create WOW! Magic for our Residents with Developmental and Intellectual disabilities out in the Community! BASIC FUNCTION :
Supports the mission of the Specialized Housing branch; to affirm the dignity of all Guests and Residents and support their right to participate in their communities as productive workers, valued neighbors and friends
SPECIFIC DUTIES :
- Facilitate contacts and linkages in the community to strengthen goals outlined in the Individual Service Plan.
- Fosters community membership and facilitates full community integration in accordance with person centered plans
- Develop data based, monthly progress reporting and quarterly progress summations
- Accurately and timely record financial information, handling Residents funds in accordance with policy
- Adhere to Specialized Housing policy and procedures regarding development and maintenance of Resident's individual records and clinical / behavioral and health documentation
- Maintain maximum safety conditions and cleanliness standards at all times
- Fosters an environment of empowerment and choice for all Residents.
- Supports, encourages and ensures that personal preferences, goals and aspirations are part of every Residents lives on a daily basis
- Oversees all monthly progress reports to ensure consistency between goals reported and objectives in the service plan and treatment plans
- Track, document and interface with local pharmacies to ensure proper medication supplies and storage for all program participants
- Provide support to Residents in the maintenance of their homes, including but not limited to cooking meals, shopping, cleaning and laundry
- Attends all required trainings and meetings
- Other duties and responsibilities as assigned
Qualifications
EDUCATION :
Degree preferred, may be waived if experience indicates competence
EXPERIENCE :
Experience working with people with developmental disabilities and experience with Autism spectrum disorders
