American Veterans Hospice is seeking an exceptional Registered Nurse (RN) and Licensed Vocational Nurse (LVN) to do visits throughout Murrieta, CA.

AVH is a locally owned company with owners actively participating in the day-to-day operations of the agency performing as part of the team. We are fully staffed and looking to expand our team in order to support and anticipate our growing patient census's needs.
Responsibilities:
- Evaluate patient/family needs in the preparation and execution of the interdisciplinary plan of care for each patient on the team and the effectiveness of treatments and monitors the patient's reactions
- Complete patient/family initial and ongoing assessments to determine hospice needs, re-assess and update the plan of care
- Provide direct skilled bedside nursing care
- Evaluate/provide/educate/maintain a safe environment for the patient
- Effectively utilize resources to assist in meeting patient's needs
- Contribute to providing hospice continuous care according to the plan of care and prescribed doctor's orders
- Participate in IDT meetings and in-services
Qualifications:
- Experience in hospice care visits is required
- Compassionate and loves to listen to / educate patients
- Proficiency in computer/ smartphone for electronic notes use
- Knowledge of HospiceMD software is required
- Bilingual is preferred
- Valid State license
- Understanding of hospice philosophy and needs of the terminally/seriously ill
- Good communication and organizational skills.
