American Veterans Hospice is looking for a compassionate, organized, and team-oriented individual to fill the role of Per Diem Hospice LVN or RN.

In this essential position, you will work closely with our Hospice Director of Nursing, clinical staff, and front office team to ensure coordinated patient care and provide support to our exceptional team of healthcare professionals. Your contributions will help us maintain our reputation for excellence and enhance the patient experience.
Our agency uses an after hours triage service where all calls are directed to, after call is screened, it is then transferred to the on-call nurse on duty that evening or weekend. The on-call hours are from 6:00pm to 8:00am. It includes weekdays and weekends. If the call warrants an actual visit (an existing patient that must be seen, a new admission, or a death call) , the on-call nurse will make the visit.
Responsibilities:
- A qualified Licensed Practical/Vocational Nurse or Licensed RN administers nursing care to terminally ill Hospice patients as needed. This is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the DPCS. Services are furnished in accordance with Hospice policies.
Qualifications:
- Experience in hospice care visits is required
- Compassionate and loves to listen to / educate patients
- Proficiency in computer/ smartphone for electronic notes use
- Knowledge of HospiceMD software is required
- Bilingual is preferred
- Valid State license
- Understanding of hospice philosophy and needs of the terminally/seriously ill
- Good communication and organizational skills.
