Qualifications

• Valid LVN license
• Proficient in patient care and assessment
• Experience with EHR systems and HIPAA compliance
• Knowledge of catheterization and tube feeding
• Basic life support certification
• Familiarity with critical care and long-term care environments
• Experience in home health or assisted living settings
• Strong communication skills and ability to work with diverse populations

Responsibilities

• Provide direct patient care in home health settings, including catheterization and tube feeding.
• Conduct patient assessments to monitor health status and implement care plans.
• Maintain accurate and confidential patient records using EHR systems while adhering to HIPAA regulations.
• Administer basic life support and IV infusion as needed.
• Collaborate with healthcare teams to ensure comprehensive care for patients, including those with developmental disabilities.
• Educate patients and families on health management and care procedures.
