Key Responsibilities
- Develop and manage care plans tailored to each client’s specific needs, conduct home visits to monitor care quality, and ensure client satisfaction through ongoing communication.
- Provide leadership and guidance to caregivers, including training and support to meet care standards. Assist in addressing performance issues and stepping in to provide direct care when needed.
- Ensure compliance with Texas Health and Human Services Commission (HHSC) regulations for Personal Assistance Services (PAS). Maintain accurate records, participate in audits, and conduct quality assurance reviews.
- Handle scheduling conflicts, caregiver absences, and other operational challenges efficiently. Navigate stressful situations calmly while maintaining focus on delivering quality outcomes.
- Foster strong relationships with clients, families, and team members to ensure seamless communication and service delivery.
Qualifications and Skills
This role requires a strong foundation in client care, problem-solving, and leadership. A CNA license or higher is required. This position also emphasizes a candidate’s ability to work independently, demonstrate empathy, and excel in a fast-paced, dynamic environment. Candidates should also have at least 3 years of experience in case management, non-medical home care, or a related field.
- Proven ability to work autonomously and guide caregivers effectively.

- Familiarity with care management software (e.g., WellSky), various apps, and office tools (Microsoft Office Suite).
