What are the responsibilities and job description for the PHENND Fellows: Health Services Community Coordinator position at AmeriCorps?
Woods is a Life Cycle Care Management Organization with more than 110 years of experience serving children and adults with intellectual and developmental disabilities. Woods is part of a System of Care that is composed of Woods Services and 12 nonprofit affiliates and related organizations based in Pennsylvania and New Jersey, employing a total of 6,600 people and serving 50,000 individuals, with 1,500 employees at its headquarters in Langhorne – all focused on the same purpose. The overarching goal and mission is to ensure that people with disabilities can live as independently as possible and access all needed services that enable them to do that. Health and wellness play a critical role in people’s ability to work, volunteer, and otherwise participate in civic life. Woods outpatient services in Langhorne, PA, collectively called Woods Healthcare, provides primary medical, behavioral, and dental health care services to community members with intellectual disability, autism, and behavioral health challenges, delivered through healthcare providers who are knowledgeable, compassionate and skilled at treating this complex population. This project is a part of the PHENND Fellows VISTA program housed at the Philadelphia Higher Education Network for Neighborhood Development (PHENND). PHENND is a network of over 25 colleges and universities that strengthens service-learning, civic engagement, and community partnership in Philadelphia.
Member Duties : The overall goal of the VISTA project is to increase access to integrated primary medical, behavioral, and dental healthcare services at Woods for community members with intellectual disability, autism, and other disabilities. The VISTA member will help accomplish this by 1) conducting outreach and raising awareness of Woods Healthcare services; 2) increasing volunteer opportunities for people with intellectual disabilities; and 3) raising additional funds through grant writing to support Woods Healthcare. We anticipate that in Year 2, the VISTA will build on the outreach database developed in Year 1, conduct community outreach, assist with setting up visits/tours, presentations, and calls, write compelling materials for both marketing and grant purposes, identify funding prospects, assist with grant-writing, and build relationships with community partners for referrals and volunteer opportunities.
