Position Overview :
The Behavioral Health Case Worker plays a critical role in managing and coordinating care for members with behavioral health conditions and complex needs. As a key member of the care team, you will be the primary point of contact for members, navigating their journey to improved health by utilizing strong assessment and communication skills, as well as clinical knowledge.
Key Responsibilities :
- Support members during transitions of care through assessment, coordination of care, education on discharge plans, and ongoing evaluations.
- Review medication lists and assist members in coordinating with community pharmacists to ensure comprehensive medication reconciliation.
- Evaluate, monitor, and update care plans through regular follow-ups based on progress and needs.
- Establish collaborative points of contact within community, medical, and behavioral health teams.
- Maintain accurate and timely documentation of member interactions in electronic care management platforms.
- Monitor service utilization and coordinate with other payer sources, making referrals as needed.
- Gain knowledge of care management programs, policies, workflows, member products, and community resources.
- May present cases during care management rounds and follow up on recommendations to achieve optimal outcomes.
- Promote a positive workplace culture by sharing knowledge and collaborating with diverse members.
- Conduct face-to-face visits at various locations, including member residences, offices, and community sites for education and assessment.
- Perform other duties as assigned.
Education and Experience Required :
Travel Requirement : Willingness to travel to members’ homes and community locations is essential. A reliable vehicle, valid driver's license, and current auto insurance are required.
