Key Responsibilities:

1. Coordinate care for members with complex needs, ensuring seamless transitions and optimal health outcomes.
2. Develop and implement care plans tailored to each member's unique situation and needs.
3. Communicate effectively with members, families, and healthcare providers to ensure comprehensive care and support.
4. Monitor and evaluate care plan effectiveness, making adjustments as needed to optimize member outcomes.
5. Document member interactions accurately and efficiently in ACFC electronic care management platforms.
6. Identify and address quality of care issues, collaborating with colleagues to develop solutions.
7. Stay informed about ACFC electronic care management platforms, care management programs, and regulatory requirements.
8. Participate in care management rounds to share knowledge and best practices.
9. Support a positive workplace culture, fostering collaboration and empathy among colleagues.
10. Conduct face-to-face visits at member residences or community locations to provide education and support.
11. Other duties as assigned.
