- Develop and implement care plans tailored to each member's unique situation and needs.
- Communicate effectively with members, families, and healthcare providers to ensure comprehensive care and support.
- Monitor and evaluate care plan effectiveness, making adjustments as needed to optimize member outcomes.
- Document member interactions accurately and efficiently in ACFC electronic care management platforms.
- Identify and address quality of care issues, collaborating with colleagues to develop solutions.
- Stay informed about ACFC electronic care management platforms, care management programs, and regulatory requirements.
- Participate in care management rounds to share knowledge and best practices.
- Support a positive workplace culture, fostering collaboration and empathy among colleagues.
- Conduct face-to-face visits at member residences or community locations to provide education and support.
- Other duties as assigned.
