AmeriHealth Caritas is seeking a compassionate and skilled Care Manager II to join our team. In this role, you will be responsible for managing and coordinating care services for members with complex medical and behavioral health needs.
The ideal candidate will possess strong assessment and communication skills, as well as the ability to work effectively in a fast-paced environment. As a Care Manager II, you will serve as the primary point of contact for the care team and guide members in achieving their optimal level of health.
Key responsibilities include:
- Supporting members during transitions of care through assessment, coordination, education, referrals, and evaluation
- Reviewing medication lists and educating members on pharmacy needs and mitigation strategies
- Developing and updating care plans through regular follow-up contacts
- Establishing points of contact to collaborate with community, medical, and/or behavioral health teams
Requirements for the role include:
- Local residence in Jacksonville, NC, to engage with members via face-to-face visits
- Master's degree in social work or current, unrestricted North Carolina LCSW license
- Associate degree in nursing required, Bachelor's degree preferred
- Minimum 3 years of professional clinical practice experience as a RN or LICSW working with adults with complex medical and behavioral health diagnoses
