Job Summary
We're seeking a talented Care Manager (RN/LCSW) to assist members in achieving their optimal level of health through self-management. The successful candidate will engage members, caregivers, and providers to assess plan and establish individual member goals. They will facilitate and coordinate care while ensuring quality and cost-effective resources.
Responsibilities
- Assess members through face-to-face encounters or by telephone to determine care coordination and care management needs.
- Complete comprehensive person-centered assessments inclusive of physical health history, mental health history, social determinants of health, and supportive needs.
- Coordinate physical, behavioral health, and social services, including medication management.
- Identify problems/barriers for care coordination and implement appropriate care management interventions.
- Create a plan of care to assist members in reducing/resolving problems and achieving their optimal level of health.
- Identify and implement the appropriate level of intervention based on the member's needs and clinical progress.
- Schedule follow-up calls as necessary, make referrals, and document progress towards meeting goals.
Requirements
- Masters Degree in Social Work.
- Active state Clinical Social Work license.
- 3 years of professional practice experience.
- Valid driver's license with car insurance.
- Experience in Case/Care Management preferred.
