Responsibilities
The primary purpose of the job is to : support the development of and executes on the outreach strategy in a territory for the purposes of impacting health outcomes, health care costs, and member engagement. This position is responsible for establishing and cultivating relationships with community stakeholders and providers, implementing and managing community events and education programs, and engaging a group of select members in that territory.
Based on the territory / market assigned, the Community Health Educator Sr may be asked to focus in one or all of the below functional areas.
Community Programs & Events :
- Leads the planning and execution of large outreach events.
- Partners with targeted community stakeholders and providers to plan and implement preventative health education programs and screening events that improve the health status of our members and ultimately health outcomes.
- Promotes member and public awareness of the importance and the availability of screenings and services for the purposes of supporting healthy lifestyles in efforts and improving HEDIS measures.
- Develops and maintains relationships with new and established community-based organizations to promote strong partnerships.
- Assist with event and sponsorship submissions to the Dept. of Health Care Finance (DCHF).
- Maintains marketing and health promotions storage units.
- Coordinate and distribute AmeriHealth Caritas DC branded apparel and items to member-facing staff.
