What are the responsibilities and job description for the Community Health Educator position at AmeriHealth Caritas?
Responsibilities
Responsible for improving the health status of our members through preventative health education / screening programs by identifying and partnering with providers and organizations to hold community events / health fairs / health education programs and screenings. In an effort to empower Plan members to take control of their health, this position will provide health education to members, parents and the community in cooperation with outside agencies / organizations and / or Providers. This position will promote member and public awareness of the importance and the availability of screenings and services in support of healthy lifestyles in efforts to improve HEDIS measures. May develop health education programs, implement and maintain already existing health education programs, collaborate with providers to hold screening events. Will conduct sessions personally. This position will be based in the field. Travel within counties in the designated zone required.
Education / Experience :
- Bachelor’s Degree in Heath Education, Public Heath, Nursing or the combination of equivalent education / work experience.
- Master’s Degree preferred.
- 1 to 3 years event planning.
- 1 to 3 years experience in health education, preferably in a public health environment that includes experience in dealing with disease specific / life style issues.
- 1 to 3 years Medicaid experience preferred.
- Medicaid managed care experience helpful.
- Valid driver’s license.
- Access to automobile w / current auto insurance required.
